What We
Charge
This is a cash-based practice and all fees are collected at the time of service by credit card or check.
All consultation fees are based on an hourly rate of $280 per hour. A valid credit card needs to be on file in your portal before all appointments.
New patient consultations require a $60 non-refundable deposit before the requested appointment will be confirmed. This amount will be deducted from the balance of your initial consultation. Your new patient deposit will be forfeited if the appointment is cancelled or rescheduled less than 24-hours before the appointment.
We Do Not Accept
Insurance
Julie Tebben, CNP does not accept any type of insurance. Some lab tests may be eligible to be billed to the patient’s insurer.
We Opt Out Of
Medicare
Julie Tebben, CNP has opted out of the Medicare program. This means all Medicare patients are responsible for consultation fees and must agree not to submit a claim (or request Julie Tebben, CNP to submit a claim) to the Medicare program, even if services are covered by Medicare Part B. All Medicare patients must review and sign the Medicare Private Contract form under the Questionnaire tab in the portal in advance of the initial consultation.
